Sounds like you may have a problem with your valves. My friend has a 98 outback and had the same exact problem. He replaced all the same things you have and the problem persisted.

Problem: MELTED VALVE in the #4 cylinder. He had to get a valve job and some other work done to repair the problem. Happened to him at 125,000 miles and I have heard this problem is common but doesn't occur in every car. Have a shop do a compression test and that will quickly give you your answer.

His did the same thing. At first, the problem would come and go. In fact, they replaced the plugs the first time and it went away, so that thought that was it. Then it would happen intermittently then it got to the point where it was permanent.

He still has the valve..it's jacked.

I don't remember exactly what caused it, but the #4 cylinder on that motor runs hotter than the others for some reason.
