I've always wondered that myself.
I've also wondered why in the hell Taco Bell doesn't offer breakfast.
They could ad 2 ingredients to their list and be open 24 hours and increase its profits and hit an entirely new target audience.
Add Eggs... and wheat tortillas. You capture the breakfast crowd and the health crowd. They do those low fat no cheese fresh salsa deals... add wheat tortillas and people would be all up on that. Especially if it was regional. Places like California would be all over that.

In n Out rocks and is the only fast food burger I would ever consider and obviously the best burgers are cooked at home. My two favorite burgers can be found in SF - Bill's Place in the Richmond and Magnolia at Haight and Masonic. Bill's place grinds their own beef and makes many different great burgers. Magnolia is a little posh but holy crap, and the best fries I have ever had at a restaurant.

The wait's are long enough as is and knowing INO, the bacon would have to be made to order.But I bet it sure was amazing!!!

Val's burger patties are also excellent. Their buns are terrible. Can't even hold the whole thing together. Man, next time I'm in So Cal, I'm getting a TOMMY BURGER. I've had Fat Burger in Vegas. It's pretty good with the fried egg. In-n-Out is good for what it is. Simple burger and fries without the crappy ingredients. There's still the Cable Car joint in SF and Barney's in Berkeley that I should try out eventually.
Anyhoo, I've seen the Good Eats episode, "Burger of the Gods". It was a good tutorial on a great standard burger, but I wouldn't go apesh*t and demanding everyone to do it, although I am a huge Food Network nerd.

The last time I was at Barney's in SF, there were baby cockroaches in the bathroom and I saw one inside the dining area. Not to mention I had mold on my bun that they claim was from the grilling process... even though it was green and furry =\
