Any idea what could cause this?

Wow... I've never seen one that bad. I've seen plenty of those idlers go out and start making noise, but none that just seize up like that. I would guess it was over tightened at some point.

i had a crank pulley separate on me and almost flew into my face wheni found it loose!

If it was over tightened, wouldnt it have failed sooner? No one has touched anything in the engine bay (besides me swapping exhaust for smog) in about 13 months

is the nut loose on the tension pulley? Could be whoever did the belt last, set the belt to tension but didn't tighten the pulley and it started to rub against the bracket and destroyed itself. or they over tightened it and the pulley proceeded to destroy itself. Just a couple thoughts.

well I dont kn ow what to say, but like paul said maybe it just failed, but I have never seen anything like this before on a car.

This bad? Not that often.
But this particular roller fails quite often....hence stretch belts.
