Help! Acceleration Problems...Clutch? 05 STi 6MT

I was cruising, not very fast, on the highway in 6th gear at a low RPM and decided to make a pass without downshifting. I pressed on the gas to pass the slow car and my car just revved to 6500+ RPM without really any acceleration. Felt like the clutch was slipping (similar to the feel of a burnt clutch on a bad launch). I decided to further investigate the problem. It seems to happen when I am any gear below roughly 3000 RPM and I stomp on the gas. The problem has never occurred before and seems to have come on all of the sudden. I can smoothly accelerate through all of the gears without slip if I accelerate hard from the start, but with a regular acceleration from a cruising speed without having to downshift my car revs and sounds like its trying to catch and go but stays in place. I have to accelerate slowly or downshift to accelerate and not have any slipping.

Is this the clutch? What else could it be?

Please help!

Car Specs: 05 STi 6MT 35,000 Daily Driver, no hard racing, never tracked. Every now and then some fun street driving. Mods: Turbo back exhaust, intake, Cobb accessport and protune. Slightly less than 300 WHP. Have not done 30k service yet...

sounds to me like a familar problem, let me guess from a dead stop take off, up throught the gears no problem, then drive along at like 2800rpms, then gun it, when it hits about 3500, it just jummps up for a bit then grabs again?
