state ref question

so I got a state ref fix-it tticket back in april. a couple months passed and i had my wife call in to file for an extension. at that time she found out that i could pay an extra amount on top of the ticket and not go to the state ref. has anyone done this before? do i get penalized later for not fixing my car, or can i just pay the extra fines and call it a day? i have to smog my car by october, and im not sure if that would affect anything with getting the car smogged.
any info is appreciated guys. TIA...
