Seattle police launched an investigation on Friday to determine how a patient undergoing emergency heart surgery caught on fire at a local hospital in 2003.
The male patient, who was not identified, went up in flames after alcohol poured on his skin was ignited by a surgical instrument.
The patient died after the surgery but that was due to heart failure and not the fire, said Dr. Robert Caplan, medical quality director of Virginia Mason.
Caplan said fires are known to occur in operating rooms although they were extremely rare.
The two-year-old incident became publicly known after an anonymous letter sent to the media mentioned it as a sign of unsafe health care at the hospital, and said the patient burned to death.
Caplan strongly disputed its contents. "That letter is factually incorrect," he said.

Do you smell smoke?
I wonder how they put it out, heh. I never would have guessed that the same patient that caught on fire during heart surgery, died from heart failure. Related?! No way!

Ok now that song is stuck in my head.
Why don't you do what you do When you did what you did to me Love was so good, that it fills up all my needs
I was crazy for you, you were crazy for me How could something so right, go so wrong My love, sweet love
Why don't you say what you say When you say what you said anymore Your eyes couldn't lie, so long goodbye, close the door
I was crazy for you, you were crazy for me How could something so right go so wrong
Tell me something I didn't do Why did someone else put you through Did they steal you away like a thief in the night Love like wings, must take flight
Why don't they play what they played On the nights you danced with me Oh, remember the one we made love to endlessly
I was crazy for you, you were crazy for me An' how could something so right go so wrong My love, sweet love
Why don't you do what you do when you did what you did to me, oh Do what you do when you did what you did to me, oh Do what you do when you did what you did to me, oh Do what you do when you did what you did to me...
