State of Hawaii Identification Card
Go to 465 South King Street, Rm 102 (corner of South King Street and Punchbowl Street)
Phone: 808.587.3112
Monday - Friday
8:00 A.M. - 2:00 P.M.
Present the following documents:
Valid passport with visa
Form I-94 (arrive/departure card in passport)
Form I-20 ID or Form IAP-66
Social Security Card
$15.00 cash
The friend of mine that got one had her birth certificate and a her SS card. It only took about five mins to get it done..
