I was just wondering if you guys/gals received a package from the Center for Disease Control/ U.S. Dept. of Health and Human Services. Contents of the Package are 2 surveys/questionaire that needs to be filled and attached to the survey is $5.00 which also is included as a payment for our time taken to fill it out. My Mom opened and I was like WTF is money in there!! The money looks real too.

 

thats because they want the surveys filled out and returned....money talks...muhahahaha

 

So what's keeping you from taking da 5 and tossing da rest?

 

I haven't received it but I know these are racially/ethnic selected. These take much time to fill out but I guarantee it is something that is quite important. It actually serves purpose for needs-assessment for both federal(CDC/DHHS) and state level(we actually use the data). They don't send it out by mass but normally are carefully selected.
