My first thought when reading was jumped timing.
You can try looking at the IAVC and see how it looks but my money is on jumped timing. You'll have to get into the timing belt cover and be able to see all the marks to find out if it has jumped a tooth or two. You may also look at the timing belt tensioner and see if its lost its tension. Should replace the tensioner everytime you do the timing belt to be safe. Is it still the same tensioner?
