So, I ran through the gears from a stop at 75-100% throttle and had no issue.

Went from a cruising speed about 3000 RPMs and hit the throttle in 4th and 5th gear and same thing, about 4000 RPMs the revs dropped about 500 RPMs and started to accelerat again. I'm perplexed...

Apply the e-brake and see if the car wants to stall while taking off. If the engine revs the clutch is gone. If it stalls then it still grabs.

From what you're describing, it's very likely the clutch. It sounds like it wants to slip at peak torque in the higher gears and then starts to catch again. The clutch is just starting to go out.
