Cusco Hyper Single Clutch

HELP!!! my hyper single has failed after just over a year. it is slipping. I need to figure out what to do. Is there a rebuild kit for this clutch (04 STi) or what clutch should I replace with for a good price?

Stock STi with TBE, Intake, Pulleys, and tune @18psi so not to high
