I opt out of the millimeter wave scanners every time. So far TSA has been timely and professional, but yes you get the most comprehensive patdown of your life. At the very least, they follow a strict script/procedure and seem to have doing it just as much as I hate having it done.
Lately, they've been defaulting to the scanners more and more over the metal detectors. I have a feeling my opt out days are numbered.
