Looks like negligence, reading from the sccaforum about this.

Driver is blaming the lugnut supplier, but c'mon... I think he needed to extend the stuf length and he didn't.

Here's the issue. I still don't believe it's the lugnuts. I believe it's the driver. I can understand if one wheel fell off. But all 4 wheels? That's saying 20 lugnuts simultaneously sheared off. Unless those lugnuts were made of plastic or butter, I doubt they all broke at the same time.
