they look like track tires. my guess is he changed them in the pit, put the lug nuts on, set car on the ground, and forgot to tighten them.

The explanation I've heard is that the lug nuts were the wrong pitch and cross threaded. Which explains all four coming off at the same time.

Even hand tightened lugs wouldn't all fail at the same time. But, if they were all finger tight, then maybe they could all fail at once.
