The DBA 4000 part number fits in the 2002 USDM caliper. There is an issue with the 2003 caliper bracket and the 4000 series disc.
Big Sky and WRX2002 have been using the DBA 4000 in the stock 2002 caliper for quite a while now.
We haven't really had a problem with any pad on the DBA 4000 disc. We've run Carbotech, Hawk, Axxis, Ferodo, Pagid, PFC, Endless, ect, ect.
My personal favourite is the Ferodo DS2500 as it is kind to the discs and the braking performance is excellent.

The only other part number is the DBA 650 which is the O.E replacement for the 16" wheel.

You mentioned that you have tried several pads for DBA rotors. I just bought the DBA slotted rotors and am curious which pads you recommend for street-occasional track use for a 02 WRX. I am leaning toward the Axxis delux rr and metalmaster frt or Pagids. What do you think?

I'm not keen on Axxis pads for track work. Auto X maybe.
They are great for the street but not really a track pad if you plan on driving hard.
*One of the Ferodo compounds is a top quality pad to consider DS2000 or DS2500. Need a bit of temperature to bring out the best but thats not hard on a WRX.
*Pagid RS4-2's are great if you have 4 pot calipers. The two pot version is hard to get.
*Hawk HPS-F ; more street orientated but can take a hiding every now and then.
*Endless CC-X another good pad.

The STD 4 pot can fit a 25mm thick (MAX) rotor and the radial clearance in the body does not clear a 324mm disc.
It can be done with some tinkering but not ideal.
