Answer to the last question is Qui'Tu also spelled Qi'Tu. On screen it's only referred to in ST:V.

That's different actually. In early versions of the original scripts (well before production) of the TOS pilot episode the ship was actually called the Yorktown not the Enterprise.
Neither did I and I think I've only seen it twice, so it figures I didn't get it either. It's funny though the guy who does the trivia said one person got the answer who just randomly decided to show up. Jen and I were the only one who knew it was going to be Star Trek night lol.
