So we have a process at my work where we provide a 'quote' or 'proposal' to our customers when they request new environments to be implemented. Once the proposal is completed, we do a peer review and you have to get three "OK's" and nothing that says the content needs modification in order to complete and submit the doc. I get all the OKs but a fourth guy says I need to modify the content and he'll forward the comments via email.
He then does not forward the comments. He goes home apparently (since he didn't return my calls until this morning). He held up my completion of the sizing (which was already delayed due to other issues) by a full extra business day all because in the analysis section there was a typo. The typo did not affect the rest of the document, the pricing, etc
WTF
