So i got a ticket...
From what my brother said, he just wrote a letter describing his situation (why he was innocent of the charge), included a few pictures, and sent that in with the forms. He probably still has it on his computer - give me a couple of hours and I'll ask him about it.

Ok, I'm looking at the letter he sent, it's pretty standard - he just pulled it out of his head. His first sentence was one that they kind of asked for, something to make the letter sound official:
He goes on to then describe the events as they happened, in detail, over the course of 5 paragraphs. He gives the date and time of the event, a description of the road conditions, when he saw the officer and what happened when he was "pulled over". Next he gave a couple of quick paragraphs as to why he believed the officer was wrong in citing him.
Finally, on the second page, he just listed/described the pictures he was including as evidence (which I suppose you won't have).
That's about it. He included the normal "letter" type things - "From", "To", "Date", and a closing/signature at the bottom. Just write the letter in a formal manner - don't make it sound like a punk 16 year old wrote it - check the spelling and grammar, all the normal things.

I declare under penalty of perjury that all information and evidence contained in this statement is true and correct. This attachment pertains to Citation Number: XXXXXX, Case Number: XXXXXX, for the defendant [Name Here].

ok so... I just got a letter back from the court saying I am found guilty for that ticket 
not sure if I want to do that trial de novo thing... not sure what to do

you can still go to court can't you? I would think this is the perfect example of why they put written declarations in place, it's a bull**** ticket and would be a waste of time (somewhat expensive taxpayer paid time) to deal with it in the courthouse. . . go figure
