lol I dont know what you mean by "imagination" it not like a resturaunt where the girls go in and say, "tell the doctor to surprise me! I like an early Michael Jackson!" 
Im sure the girls pay for and get exactly how they want to look. That said, Im sure they are not all intentionally trying to look the same, but if you have a population of girls that look more or less the same and then start taking away the small differences that make them unique (making big or wide noses smaller, lifting eyelids to make bigger eyes, making thin lips fuller) it just creates a certain amount of uniformity.
As far as racist jokes goes, the site I linked to and all the blogs Ive seen laughing about this have been Japanese.
So just to be clear, even people in Japan are making fun of a country where all the girls look even more alike than in Japan.

There are a lot of woman who go in asking for the surgeons 'creative insight.' Yes quite a few going in just ask "Nip this and tuck that" but even still the doctor will give his opinions. Then the surgeon has sex with them, at least thats what Nip/Tuck taught me.

If you look in music for interest, their top stars. One is straight Korean, another is Korean and Black but they speak and live in Japan.
