No, just the knob. The shift trim is from: http://www.autospeedperformance.com/

Easy. Twist off the original knob. Place the rubber insert over the threads on the stick. Place the knob on the stick. Tighten the 3-hex head screws and screw on the trim below the knob.

You can also order the "Paulie adapter" from a guy here on the boards. It's a plastic sleeve that threads on like stock that holds the factory shift boot to the Momo knob, otherwise you might have to do something to keep the boot up. The adapters run about $20 or so. I have one but I am keeping it in case I decide to get another knob later on. Search the boards in case you want to mod something on your own. I've seen guys attach rubber and zip tie under the boot to keep it from faling.
